Digital interventions to support people with multiple health conditions
Abstract
Multimorbidity, suffering from two or more chronic health conditions, poses a global challenge impacting individuals, carers and society. While many chronic conditions are manageable through self-care, those with multimorbidity face difficulties prioritising conditions, managing medications, and adhering to lifestyle changes.
This research aims to explore the needs, priorities, and challenges of individuals with multimorbidity (and other key stakeholders) in understanding and managing their conditions, proposing digital interventions for support.
This project will benefit from collaborations with the NWL Diabetes Team at Imperial and the Stroke Research Patients and Family Group based at King’s College London.
