AB - Alcoholic hepatitis (AH) is a clinical syndrome characterized by recent onset of jaundice in actively drinking patients. The severe form of the condition is associated with significant short-term mortality. General measures that are important in the management of the condition include abstinence from alcohol with treatment of withdrawal and prophylaxis against Wernicke's encephalopathy, nutritional assessment and appropriate therapy, and aggressive screening and treatment of infection. The two main treatment options at the present time are pentoxifylline and corticosteroids. Controversy surrounds the evidence for their efficacy but currently supports a role for their use in the treatment of severe AH. Evidence exists to suggest a role for early liver transplantation in AH; however, it requires further evaluation and the topic is certainly controversial. New therapeutic strategies are likely to try to combat steroid resistance or target other pro- and anti-inflammatory pathways.
